STEP 1: Confirm Your Clients Eligibility
The following are requirements for purchase:
- Membership in Live Well USA is required to access benefits, including insurance policies,
- Fees: $19.99 monthly membership fee for joining the Live Well group. The Live Well Membership fee is non-refundable.
- Employment: Members must be gainfully employed and actively working a minimum of 20 hours per week.
- Affirmations:
- I affirm that I am not currently terminally ill, hospitalized, residing in a nursing home, assisted living facility, or under hospice care.
- I affirm that, to the best of my knowledge and belief, I have not been diagnosed with Acquired Immunodeficiency Syndrome (AIDS), AIDS-related complex (ARC), or tested positive for Human Immunodeficiency Virus (HIV).
